Harnessing Data, Improving Patient Care

Healthcare providers frequently cannot see the complete medical histories of patients, particularly those patients who move through the healthcare system and receive care from disparate providers. The Centers for Medicare and Medicaid Services (CMS) is meeting that challenge by providing a more complete picture of patient profiles and histories. A new CMS pilot program, Data at the Point of Care, uses claims data to fill gaps in patients’ medical histories. The program’s goal is to support healthcare providers by giving them easy access to consolidated patient data, including secure, structured patient histories; past procedures; and medication adherence, among others.
